Please complete this contact form. Upon receipt, we will call you to discuss your desired goals and ensure we can provide the type of therapy you are requesting. We will then email you a link to sign up for our online Client Portal where you will complete all of our intake paperwork. Once this information is entered into our system, we can schedule your first appointment.

Wellness Connection of Iowa currently accepts Wellmark of Iowa- Blue Cross Blue Shield and Optum- UHC insurances. If your insurance is not accepted, please contact us to see if we are able to credential with your company. If not, you have the option of private paying for appointments. Please contact us for more information on our fee schedule.
Payment and insurance copayments are required at the time of the appointment. During the intake process, you will be asked to add a credit card to your account. All copayments and costs not covered by insurance will be charged to this card.
We ask that you provide us with 24-hour notice if you need to cancel an appointment. You will be charged a cancellation fee of $50 if you do not provide notice.
